* $50 non-refundable deposit required for all appointments.
* Deposit will be deducted off the total.

* A Credit Card must be kept on file to book an appointment 
* Please read cancellation policy prior to booking.
* 10-minute grace period, appointment is cancelled after 10 minutes
* We are child-free Wellness Clinic (for their safety and insurances purposes)

I Authorize Enrich Health & Wellness to save this card on file for future purchases until you cancel this authorization. Purchases will never be made without your approval. Saved cards can be managed at any time through your profile page.

**Appointment Booking Guidelines**  

For both new and returning patients, a non-refundable deposit of $50.00 is required at the time of scheduling your appointment. This deposit permits one ​adjustment to your appointment. After this change, the original $50.00 deposit will be forfeited, and a new deposit will be necessary for any subsequent appointments. If you cancel or fail to attend your scheduled appointment, the deposit will be lost and cannot be credited to future visits.*ALL DEPOSITS ARE NON REFUNDABLE** By making this and all deposits, you consent to Enrich Health & Wellness to file and charge the credit card according to the terms outlined in this agreement. This payment authorization covers goods, services, and healthcare provided by Enrich Health & Wellness for the amount of $50.00. I confirm that I am an authorized user of this credit card and agree not to dispute thecharge with my credit card provider, as long as the transaction adheres to the terms set forth in this policy.



**Cancellation and No-Show Policy**  

Please ensure you confirm your appointment through the texts, emails, or phone calls sent by Enrich Health & Wellness. If you do not notify us to cancel or reschedule at least 24 hours before your appointment (be sure to leave a voicemail if needed), or if you do not show up without any communication, your deposit will be forfeited. If no deposit has been made, a $50.00 no-show fee will be charged to your credit card on file on the appointment date. A new deposit will be required for any future services. Should you have two or more no-shows within a 365-day period, a non-refundable deposit of $100.00 will be necessary before booking any further appointments.
