I am aware of the clinic's cancellation policy, and I understand the following:

The clinic is located at 421 N Rodeo Dr, Suite STE-1, Beverly Hills, CA 90210 (STREET LEVEL).
Parking is available at 9510 Brighton Way, with 2 hours of free parking. After 5pm, there is a $5 flat rate.

There is a strict 15-minute grace period for appointments.
If I need to cancel, it is my responsibility to inform the clinic as soon as possible.
Any appointment cancelled within 24 hours is subject to a $75 late cancellation fee.

I understand that all sales are final and agree that there are no refunds offered by the clinic for services rendered.

ARBITRATION: All disputes arising out of this agreement or any others, shall be submitted to mediation in accordance with the rules governing standard mediation. In the event the parties do not resolve the dispute through mediation, those unresolved disputes shall be submitted to binding and final arbitration. The arbitrator shall be determined by the customary three strikes rule and the arbitration shall occur within Los Angeles County. The Arbitrator's award shall be final with judgment entered thereon in the court of jurisdiction. The laws of the State of California shall apply to any legal proceeding. The prevailing party shall receive costs of suit and attorney's fees.
