Booking Terms & Conditions
Appointments and Cancellations:
Appointment Scheduling:
Appointments can be scheduled by phone, email, or through our online booking system. We recommend scheduling in advance to secure your preferred date and time.
*New Clients, please aim to arrive 5-10 minutes before your appointment to allow for check-in and paperwork.
If you are running late, please call us at (847) 693-4663. We’ll do our best to accommodate you, but your appointment may be shortened to avoid delays for other guests.
Cancellation Policy:
A minimum of 24 hours’ notice is required for cancellations or rescheduling. Cancellations made within 24 hours of the scheduled appointment may incur a $50 cancellation fee.
No-Show Policy:
Failure to attend a scheduled appointment without prior notice will result in a no-show fee of $50 per occurrence.
Payment Terms
Accepted Payment Methods:
We accept cash, credit/debit cards, Cherry, CareCredit, Apple/Google Pay, Venmo, and Zelle. Payment is required at the time of service unless otherwise arranged.
Refund Policy:
All sales on services, packages, and products are final. If you have concerns about your treatment, please contact us so we can address them promptly.
In some cases, unused packages may be exchanged for other services of equal value.
Medical and Health Information
Disclosure of Health Information:
Clients are required to provide accurate and complete medical and health history before receiving any treatments. CN Medical Aesthetics & Wellness is not responsible for any adverse effects or complications resulting from undisclosed medical conditions, allergies, or medications.
Consultations:
Certain treatments require a consultation to determine eligibility. During this visit, our providers will discuss risks, benefits, alternatives, and expected results.
Service Limitations
Right to Refuse Service:
CN Medical Aesthetics & Wellness reserves the right to refuse service to any individual if it is determined that the treatment is not in their best interest or if the client is not a suitable candidate.
Treatment Results:
While we strive for exceptional results, individual outcomes vary. CN Medical Aesthetics & Wellness makes no guarantees regarding specific treatment results.
Age Requirements:
Clients must be at least 18 years old for most services. Minors may be treated with parental consent, depending on the service.
Liability and Waiver
Assumption of Risk:
By receiving services or products from CN Medical Aesthetics & Wellness, clients acknowledge and assume all risks associated with the treatments provided.
Waiver of Liability:
Clients agree to release CN Medical Aesthetics & Wellness, its owners, employees, and contractors from any and all claims or damages arising from services rendered, except in cases of gross negligence or intentional misconduct.
Privacy and Confidentiality
Client Information:
Your privacy is our priority. All personal, medical, and financial information shared with CN Medical Aesthetics & Wellness will be kept strictly confidential and used only for treatment and administrative purposes.
Data Protection:
CN Medical Aesthetics & Wellness complies with all applicable Illinois and federal privacy and data protection laws.
Follow-Up Policy for Aesthetic Procedures
At CN Medical Aesthetics & Wellness, client safety and satisfaction are our highest priorities.
All clients are recommended to attend scheduled follow-up appointments after receiving aesthetic treatments to ensure the quality, safety, and integrity of all procedures performed in our clinic. This also includes taking photos to monitor progress.
Updates to Terms and Conditions
CN Medical Aesthetics & Wellness reserves the right to update or modify these terms and conditions at any time without prior notice. Clients are encouraged to review this page periodically to stay informed of any updates.
Governing Law
These terms and conditions are governed by and construed in accordance with the laws of the State of Illinois. Any disputes arising from these terms shall be resolved in the courts of Illinois.

By using our services and/or purchasing our products, clients acknowledge that they have read, understood, and agreed to these Terms and Conditions.
