As you schedule your appointment with our providers please keep the following in mind:
- If booking a consultation with Dr. Lieman there is a $200 consultation fee. One of our team members will contact you for payment before your appointment date.
- If you've scheduled for a BHRT consultation please note that labwork will be required for your appointment. A member of our team will reach out to you to schedule this appointment. Please allow at least 3 days from when we can get you in for your labs to your consultation appointment. *This may result in having to reschedule your consultation.
- We have a 24-hour cancellation policy: you will be subject to a $100 late cancellation/no-show fee to your card on file if an appointment is canceled within less than 24 hours.
- We do not issue refunds once services have been purchased or rendered. You may, at the discretion of Elite Medspa, be issued credit to be used within three (3) months.
