STEP 1: COMPLETE THE FREE INTAKE QUESTIONNAIRE  

• Complete the Questionnaire for IV Therapy or Weight Loss Injections. CHECK YOUR EMAIL. 
• If approved, you will be redirected to the Qualiphy.me Telehealth  link. CHECK YOUR EMAIL. 
• If redirected for follow-up, check your email or call 480-462-3882 to consult with the NuSerenity Med Team.  

 

STEP 2: PAY DEPOSIT

• New Patients. Evaluations A $50 deposit and Consent forms must be completed before your appointment is scheduled and to receive services.  
• Existing Patients. Follow-up evaluations are $27.99. 

 

STEP 3: LOG IN  TO THE QUALIPHY.ME LINK  

• CHECK YOUR EMAIL FOR THE QUALIPHY.ME TELEHEALTH LINK
• GFE's will take place via Telehealth Visit with a Nurse in the convenience of your home, work or the NuSerenity medspa.
• Walk-ins, no worries. The GFE Questionnaire can be completed from your mobile or in a consultation room.  

 

STEP 4: E-SIGN NOTICES AND CONSENT 

Notice. In Arizona all patients receiving weight loss injections and IV Therapy treatments are required to receive a Good Faith Exam (GFE) to determine their eligibility for treatment. 

Notice. The initial weight loss evaluation requires recent lab work consultations to ensure patient safety and suitability for the treatment.
