Credit Card Requirement

A valid credit card is required at the time of booking through our online platform. Your card will not be charged unless it aligns with your independent member cancellation or no-show policy.

Please note that all providers practicing at Elevate Aesthetics are members to Elevate, and independent business owners and are not employees, agents, or representatives of Elevate Aesthetics. Each provider operates their own individual business and is solely responsible for the care and services they provide.

Elevate Aesthetics assumes no responsibility or liability for the acts, omissions, or services rendered by any independent provider. By signing below, you acknowledge that you understand and agree that any treatment, advice, or care you receive is the sole responsibility of the individual provider.

Each independent member of Elevate Aesthetics has complete control over their practice, including but not limited to: services offered, marketing of their services, client list, member training, client scheduling, service pricing, collecting the cost of services for clients, client cancellation policy, and processing of client payment transactions.
