Welcome to The Maxim Clinic – Norcross. To ensure a seamless, elevated experience, please review the following terms before securing your appointment.

1. Appointment Scheduling & Confirmation

  • You may book online, by phone, email, or text.
  • A confirmation and calendar invite will be sent once your booking is complete.
  • Please verify your contact details to receive timely reminders.

2. Cancellation & Rescheduling

  • Notify us at least 1 hour before your appointment to cancel or reschedule via phone, email, or text.
  • Late cancellations may incur a fee or forfeit your deposit.

3. Deposits

  • Certain bespoke services require a non-refundable deposit at booking.
  • Your deposit applies directly to your treatment cost on the day of service.

4. Late Arrivals

  • Arrivals beyond 15 minutes may require rescheduling to preserve your full experience.
  • If you anticipate a delay, please contact us immediately.

5. New & Returning Patients

  • New patients receive a comprehensive consultation—either at your first visit or as a separate session.
  • Returning patients without a visit in the last 12 months require a brief follow-up consultation.

6. Informed Consent

  • Prior to treatment, you will review and sign an informed-consent form outlining procedure, benefits, and potential risks.
  • Disclose your full medical history and allergies to ensure your safety.

7. Pre-Treatment Preparation

  • Specific preparations may apply (e.g., avoiding certain medications or sun exposure).
  • Detailed instructions will be provided at booking.

8. Post-Treatment Care

  • Aftercare guidelines support optimal results and minimize complications.
  • We are not liable for outcomes if aftercare instructions are not followed.

9. Results & Expectations

  • Individual results vary based on physiology, lifestyle, and adherence to guidance.
  • Multiple sessions may be recommended for optimal outcomes.

10. Medical Clearance

  • Certain treatments require physician approval, especially if pregnant or managing chronic conditions.
  • We reserve the right to defer or decline treatment if medically inappropriate.

11. Minors

  • Clients under 18 must be accompanied by a parent or guardian, who will co-sign consent forms.

12. Allergy Disclosure

  • Disclose all known allergies during consultation.
  • We are not liable for reactions if allergies are undisclosed.

13. Refund & Package Policy

  • Services are non-refundable once rendered.
  • Pre-paid packages may be transferable at management’s discretion; please inquire.

14. Payment Terms

  • Full payment is due at the time of service unless alternative arrangements are confirmed.
  • Accepted: major credit/debit cards, Zelle, Venmo, cash.
  • In-house financing via CareCredit is available.

15. Special Offers & Promotions

  • Offers cannot be combined unless expressly stated.
  • Promotions are subject to availability and may be modified or withdrawn.

16. Health & Safety Protocols

  • We uphold rigorous hygiene and sanitation standards.
  • Clients exhibiting illness symptoms will be asked to reschedule.

17. Service Termination

  • We reserve the right to refuse or discontinue services if safety or compliance concerns arise.

18. Dispute Resolution

  • Any disputes will be resolved by arbitration under Georgia law, rather than litigation.
