Terms and Conditions for Online Booking for Doctor Wellness & Weightloss Studio of Norcross
Welcome to Doctors Wellness & Weight-Loss Studio of Norcross!
To ensure the best experience for our clients and the safety of our patients, we ask that you carefully review the following terms and conditions before booking your appointment.
1. Appointment Booking and Confirmation
- All appointments can be scheduled through our online booking system, telephone, email or text.
- You will receive a confirmation email or text once your appointment is successfully booked.
- Please ensure your contact information is accurate to receive appointment notifications.
2. Cancellation and Rescheduling Policy
- Please provide us within the hour of notice to cancel or reschedule either by phone, email, or text.
3. Deposits
- Certain treatments require a deposit.
- This deposit will be applied toward your service on the day of your visit.
4. Late Arrivals
- If you are more than [15] minutes late, your appointment may need to be rescheduled, please contact the office location where your appointment is scheduled either by phone, email, or text.
- If you anticipate being late, please contact us as soon as possible.
5. New Patients
- A consultation is required before certain treatments. This can be conducted during your first visit or scheduled separately.
6. Existing Patients
- If you have not visited us in the past 12 months, a consultation with your provider is required prior to booking further treatments.
7. Informed Consent
- Prior to any treatment, patients will be required to sign an informed consent form. This form will outline the nature of the treatment, potential risks, and expected outcomes. By signing, you acknowledge understanding the information provided and agree to the procedure.
- Failure to disclose relevant medical conditions or allergies may result in adverse outcomes.
8. Pre-Treatment Instructions
- Certain treatments may require specific preparation (e.g., avoiding certain medications, refraining from sun exposure). These instructions will be provided when booking your appointment.
- Failure to follow pre-treatment instructions may impact your results.
9. Post-Treatment Aftercare
- Aftercare instructions will be provided after your treatment. It is your responsibility to follow these guidelines to ensure optimal results and minimize the risk of complications.
- We are not liable for adverse outcomes if aftercare instructions are not followed.
10. Results Disclaimer
- Individual results may vary depending on health, skin type, lifestyle, and adherence to pre- and post-treatment instructions.
- Multiple sessions may be required to achieve desired results.
11. Medical Clearance
- If you have a medical condition or are pregnant, certain treatments may require clearance from your physician.
- We reserve the right to refuse or defer treatment if it is deemed medically unsafe or inappropriate.
12. Minors
- Patients under the age of 18 must be accompanied by a parent or legal guardian for consultations and treatments. The parent/guardian must sign all consent forms on the minor’s behalf.
13. Allergy Disclaimer
- Please inform us of any known allergies during your consultation. We are not responsible for adverse reactions if relevant allergies are not disclosed.
14. Refund Policy
- All services are non-refundable once rendered. However, if you are not satisfied with your service, we may offer a credit toward future treatments at management’s discretion.
- Pre-paid treatment packages are non-refundable but may be transferable to other services or individuals upon request.
15. Payment Terms
- Payment in full is required at the time of service, unless alternative payment arrangements have been mutually agreed upon in advance.
- We accept major credit cards, debit cards, Zelle, Venmo, and cash payments.
- Our services are not typically covered by insurance, and we do not file insurance claims on behalf of patients.

16. Promotions and Special Offers
- Promotions and offers cannot be combined with other discounts unless otherwise stated.
- Offers are subject to availability and may be withdrawn at any time.
17. COVID-19 Protocols
- For your safety, we adhere to strict hygiene and sanitation protocols.
- Clients exhibiting symptoms of illness may be asked to reschedule their appointment.
- Please review our current COVID-19 policies before your visit.
18. Termination of Services
- We reserve the right to refuse treatment or terminate services if we determine it is unsafe or inappropriate for the patient.
- This includes non-compliance with treatment protocols or aftercare instructions.
19. Dispute Resolution
- Any disputes arising from treatments will be resolved through arbitration in accordance with the laws of Georgia.
- By booking a service, you agree to resolve any disputes through this process rather than pursuing litigation.
