Cancellation and No Show Policy
ALiGN has a 48-hour cancellation policy. I understand that if I need to change or cancel my appointment, I must notify the practice via the office text line at (754) 254-4663 at least 48 hours prior to the scheduled appointment time. Late cancellations and no-shows are charged the full service fee per our Cancellation Policy, with a minimum charge of $250.
Refund Policy
Prescription medications and medical grade supplements are subjected to state and federal laws which do not permit us to restock medical items or to accept returned prescriptions. I understand that all sales of ALiGN, including but not limited to all appointment fees, services, products and gift certificates, are final.
Practice Policies ALiGN requires keeping a valid credit card on file to schedule any appointment. All payments are expected at the time of service and/or product ordering. We only accept payment in the form of Visa, Mastercard, or American Express. Cash and personal check may be accepted if the patient is present in office and not remote/telehealth. Cherry Pay may be used if pre-approved in advance and paid for at the time services are rendered. The patient’s card on file will be charged for any services received, programs enrolled in and products purchased at (or after) the time of service.
Consultation Fee
Please note the cost of a BHRT or Weight Loss Intake is $250 and a Functional Psychiatry Intake is $500 and must be paid in full on the day services are rendered, whether or not you decide to move forward with a program.
IMPORTANT INFORMATION FOR HORMONE AND WEIGHT LOSS PATIENTS:
We require bloodwork before your appointment.
This must be done 4-5 days before your scheduled intake, to ensure the results have been processed and reviewed. Please ensure that your appointment is at least 5 days away - if not, please choose a different day.
We will send your lab order to Quest upon booking.
