Telemedicine Appointment Terms and Conditions
By booking a telemedicine appointment through our online portal, you agree to the following terms and conditions. Please read them carefully before proceeding with your booking. These terms apply to all telemedicine consultations.

All personal information provided during the booking process and consultation will be treated confidentially and in accordance with applicable data protection laws, including HIPAA. We use secure technology to ensure that your information remains private.
Consent to Telemedicine
By booking this appointment, you consent to receiving medical care through telemedicine, which involves the use of electronic communications (e.g., video, phone) to provide consultation, diagnosis, treatment, and other healthcare services. You understand that telemedicine consultations may differ from in-person appointments and may have certain limitations.
Appointment Scheduling and Cancellation
You can schedule a telemedicine appointment using our online booking system. If you need to cancel or reschedule, please provide at least 24 hours’ notice. Missed appointments or cancellations with less than 24 hours’ notice may incur a cancellation fee.
Technical Requirements
It is your responsibility to ensure you have a stable internet connection and appropriate devices (e.g., smartphone, tablet, computer with webcam) to conduct the telemedicine consultation. We are not responsible for issues arising from technical failures on your part.
Limitation of Telemedicine Services
Telemedicine appointments are not suitable for emergency situations or conditions requiring immediate attention. If you are experiencing a medical emergency, please contact emergency services immediately, by calling 911.
Consultation FeeThe consultation fee is due in full at the time of booking and is non-refundable. This fee may be applied towards a treatplan, as recommended by your provider during the consultation. Please ensure that you are aware of these terms before proceeding with your appointment.ment.
Medical Records
Your medical records from telemedicine consultations will be maintained as part of your overall medical record with our practice. You may request copies of these records at any time, subject to applicable privacy laws.
Limitations of Liability
While we strive to provide the highest standard of care through telemedicine, we are not liable for any limitations or issues arising from the use of telemedicine platforms, including delays or miscommunication. You understand and accept these limitations as part of your consent to telemedicine services.
Governing Law
These terms and conditions are governed by the laws of Florida, and by agreeing to them, you submit to the jurisdiction of the courts of Florida.
