These Terms & Conditions outline the guidelines for using our website, booking appointments online, purchasing products, and booking and receiving services at Rejuvaline MedSpa.

By visiting our website, booking an appointment online or by phone, purchasing a product or service, or otherwise engaging with Rejuvaline MedSpa, you acknowledge that you have read, understood, and agree to these Terms & Conditions.

We encourage you to take a moment to review these terms so you can feel confident, informed, and comfortable throughout your Rejuvaline experience.

Payment & Financial Responsibility Policy

Rejuvaline MedSpa

We want your experience at Rejuvaline MedSpa to be simple, comfortable, and transparent—including payment. This policy explains how payments, cancellations, packages, refunds, HSA/FSA eligibility, and other financial responsibilities are handled.

By scheduling an appointment or purchasing services or products, you agree to the terms below.

1. Payment at the Time of Service

Payment is due when services are provided unless you have an approved payment plan or treatment arrangement in advance.

Retail products must be paid for at the time of purchase.

2. Accepted Payment Methods

We accept major credit and debit cards, cash, and other payment methods approved by Rejuvaline MedSpa.

3. Card on File

To make scheduling easier and help secure your appointment, we may ask you to keep a valid credit or debit card securely on file.

By providing a card on file, you authorize Rejuvaline MedSpa to charge it for:

  • Services not paid for at the time of treatment
  • Approved treatment package charges
  • Outstanding balances
  • Applicable late cancellation or no-show fees
  • Other charges you have agreed to

Please keep your payment information current. You are responsible for updating your card if your payment information changes.

4. Declined or Unsuccessful Payments

If a payment is declined, reversed, disputed, or otherwise unsuccessful, you are responsible for resolving the balance promptly.

Outstanding balances may need to be paid before additional services are provided or future appointments are scheduled.

 

5. Cancellations and No-Shows

We understand that plans can change. If you need to cancel or reschedule your appointment, please provide advance 24 notice.

We will charge a $50.00 fee for cancellations made without the required advance notice and for missed appointments without notice (no-shows). The applicable fee may be charged to the credit or debit card on file.

By scheduling an appointment, you acknowledge and agree to Rejuvaline MedSpa’s Cancellation & No-Show Policy.

 

6. Treatment Packages

Treatment packages must be paid according to the terms provided at the time of purchase.

Packages are generally intended for the person who purchased them and may have expiration dates, transfer restrictions, or other conditions.

Unused package services are subject to our package and refund policies.

7. Payment Plans and Financing

If you choose an approved payment plan or financing option, you are responsible for making all payments according to the agreed-upon terms.

Services may be paused if required payments are not made as scheduled.

8. Refunds and Credits

Refunds, credits, exchanges, and adjustments are handled according to Rejuvaline MedSpa’s applicable policies and the terms associated with the service, package, promotion, or product purchased.

Services that have already been performed are generally not refundable. Some prepaid treatments, packages, promotional purchases, and products may also be non-refundable.

Please contact our team if you have questions about a specific purchase.

9. Product Returns

Retail product returns and exchanges are subject to our Product Return Policy.

Products must meet all applicable return requirements, including any time limits and condition requirements.

10. Insurance, HSA, and FSA

Rejuvaline MedSpa is primarily a private-pay practice.

Certain services or products may be eligible for reimbursement through an HSA or FSA, depending on the patient’s plan, the nature of the service, and applicable plan requirements. Eligibility is determined by the patient’s HSA or FSA administrator, not by Rejuvaline MedSpa.

Patients are responsible for confirming eligibility and understanding any documentation requirements before receiving services or purchasing products. Rejuvaline MedSpa does not guarantee reimbursement, eligibility, or tax treatment.

When appropriate and upon request, Rejuvaline MedSpa may provide a Letter of Medical Necessity (LMN) or other supporting documentation for submission to an HSA or FSA administrator. Providing an LMN does not guarantee approval or reimbursement. The patient is responsible for submitting the documentation and responding to any requests from the HSA or FSA administrator.

Patients are also responsible for determining whether services or products may qualify for reimbursement through insurance. Reimbursement and eligibility are not guaranteed by Rejuvaline MedSpa.

11. Your Financial Responsibility

You are responsible for all charges related to services, treatments, products, packages, payment plans, and other purchases made through Rejuvaline MedSpa.

If an HSA, FSA, insurance plan, or other third-party payer denies or does not reimburse a charge, you remain responsible for the full balance.

If you have questions about pricing, payment options, HSA/FSA eligibility, LMN requests, or this policy, please speak with our team before receiving services or making a purchase.
