Complimentary Consultations 

We are pleased to offer comprehensive, complimentary 45-minute consultations for new clients!

 

Upon booking you will recieve two emails: an appointment confirmation, as well as an intake form.

Please note, it may take up to 1 hour to recieve the intake form.

 

These e-mails will be sent from "Quantum Laser Lab" notices@envisionalerts.com. 

 

Be sure to check your junk/spam folder and add us to your contacts or safe sender list to ensure our emails land in your inbox! 

 

The intake form must be completed at least 24 hours prior to your appointment in order to hold the complimentary appointment.  

 

*If the intake form is not completed within 24 hours of the appointment and we are unable to contact you, we reserve the right to cancel the appointment. 

 

Late Cancellation & No-Show Policy

We require at least 24 hours notice for consultation cancellations or rescheduling.

Cancellations made within 24 hours of your scheduled consultation, or failure to attend (no show) will result in a non-negotiable $75 (plus tax) fee.

This fee must be paid in full prior to rebooking.
Payment can be made via credit card, or e-transfer.

 

Please note a credit card will be required for any treatment bookings made going forward.
