Thank you for booking your appointment online with Evolution. Please review the following terms before confirming your booking:

Appointment Confirmation
Your appointment is not confirmed until you receive a confirmation SMS from us. We reserve the right to reschedule or decline bookings if necessary. In the case that your selected provider becomes unavailble we may change your appointment to a different provider and plan to update you when necessary.

Payment Policy
Our clinic does not accept insurance or payments made by check or money order. We do accept HSA and FSA. We also accept Cherry and Care Credit. Consultations are currently $50.00. All appointments require payment unless provided in an agreed program. Currently our Aesthetic Consultations are complimenatary. Payments will be due on the day of your appointment unless otherwise notified.

Cancellation and No-Show Policy
We kindly ask for at least 24 hours’ notice for any cancellations or rescheduling. Late cancellations or missed appointments may incur a cancellation fee of $50.

Late Arrivals
If you are late, we will do our best to accommodate you, but your appointment may need to be shortened or rescheduled. Arriving more than 15 minutes late may result in appointment cancellations and additional fees. Please notify our staff if you plan to be more than 15 minutes late.

Medical Emergencies
If you are experiencing a medical emergency, please call emergency services or go to the nearest emergency department.
