Thank you for booking your appointment online with Evolution. Please review the following terms before confirming your booking:

Appointment Confirmation
Your appointment is not confirmed until you receive a confirmation SMS from us. Please respond "Start" to receive appointment confirmations. Please also check your email for any forms that may need completion prior to your appointment.
*We reserve the right to reschedule or decline bookings if necessary. In the case that your selected provider becomes unavailble we may change your appointment to a different provider and plan to update you when necessary.

Payment Policy
Our clinic does not accept insurance or payments made by check or money order. We do accept HSA and FSA. We also accept Cherry and Care Credit. Payments will be due on the day of your appointment unless otherwise notified.
New Patient Consultations with Dr. Braun and Ozone/EBOO Consulations with Brandon Johnston are currently $95. If you would like a comprehensive lab panel with review please reach out to our team to schedule a lab draw.
Skin Health Consultations with Heather Schreppel are currently $35. That amount can be applied towards any skincare product within 30 days of your consultation.

Cancellation and No-Show Policy
We kindly ask for at least 24 hours’ notice for any cancellations or rescheduling. Late cancellations or missed appointments will only be accepted one time. After, the total cost of the appointment will be charged.

Late Arrivals
If you are late, we will do our best to accommodate you, but your appointment may need to be shortened or rescheduled. Arriving more than 15 minutes late may result in appointment cancellations and additional fees. Please notify our staff if you plan to be more than 15 minutes late.

If you are experiencing a medical emergency, please call emergency services or go to the nearest emergency department.
