Terms and Conditions for Appointment Requests

By submitting an appointment request with CHMD, you acknowledge and agree to the following terms:

  1. Appointment Request Only: Your submission is a request for an appointment and does not guarantee confirmation of the requested date and time.
  2. Verification Process: A member of our team will contact you to verify and confirm the appointment details. Please ensure your contact information is accurate and up to date.
  3. Office Hours: Appointment requests are processed during regular office hours. Requests submitted outside of these hours will be addressed on the next business day.
  4. Cancellation Policy: Once your appointment is confirmed, any changes or cancellations must be communicated to our office at least 24 hours in advance. Failue to do so could result in a $35 cancellation/no show fee.
