Peak Personalized Healthcare Booking Terms & Conditions
1. Appointment Booking and Confirmation
All bookings for consultations, IV drips, VO₂ max testing, DEXA scans, or any services provided by Peak Personalized Healthcare must be scheduled online or directly through our staff.
Confirmation of your booking is subject to availability and receipt of full payment or deposit where applicable.
2. Cancellations and Rescheduling
Cancellations or rescheduling requests must be made at least 24 hours prior to your scheduled appointment to avoid cancellation fees.
Late cancellations or no-shows will incur a cancellation fee equivalent to 50% of the service fee.

Please arrive promptly for your appointment. Late arrivals may result in reduced consultation or treatment time.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled, and the cancellation policy will apply.
5. Health and Medical Information
Clients must accurately disclose all relevant medical history and health conditions to ensure safety and efficacy of treatment.
Peak Personalized Healthcare reserves the right to refuse treatment based on medical assessments.
6. Liability Waiver
Clients acknowledge that all medical procedures carry inherent risks. By booking and attending your appointment, you agree to release Peak Personalized Healthcare from any liability arising from treatments, except in cases of negligence.
7. Data and Privacy
Personal data collected will be securely stored and used solely for the purpose of providing healthcare services.
We comply fully with HIPAA guidelines and privacy laws to protect your confidentiality.
8. Changes to Terms
By completing your booking, you agree to these terms and conditions.
