Terms and Agreements
By scheduling an appointment with Restorative Health of Colorado, you acknowledge and agree to the policies outlined below.
Treatment recommendations are based on the information provided during your consultation and evaluation. Results may vary from person to person, and no specific outcome can be guaranteed.
Patients are responsible for providing accurate medical history, medication information, and any updates to their health status. Failure to disclose relevant medical information may affect treatment eligibility and outcomes.
Payment is due at the time services are rendered unless otherwise arranged. Prices, services, and packages are subject to change without notice.
Restorative Health of Colorado reserves the right to refuse service when treatment is deemed medically inappropriate, unsafe, or outside the scope of practice.
By booking an appointment, you consent to receive appointment reminders and clinic communications via phone, text message, or email.

Cancellation Policy

At Restorative Health of Colorado, we value both your time and the time reserved specifically for your appointment. To help us provide exceptional care to all patients, we kindly request at least 24 hours' notice if you need to cancel or reschedule your appointment.

Late Cancellations & No-Shows
Appointments canceled with less than 24 hours' notice, missed appointments, or no-shows will be subject to the following fees:

  • Hormone Therapy Consultation: $100

  • Weight Loss Consultation: $100

  • Botox/Xeomin Appointment: $50

  • Wellness Injection Appointment: $50

  • IV Therapy Appointment: $50

These fees help offset the time reserved exclusively for your appointment that cannot be offered to another patient on short notice.

Late Arrivals
Patients arriving more than 15 minutes late may be asked to reschedule their appointment to avoid disrupting the schedule of other patients. If the appointment cannot be accommodated, the applicable late cancellation fee may apply.

Future Scheduling
Patients with a history of missed appointments or late cancellations may be required to keep a credit card on file or prepay for future appointments.

We appreciate your understanding and cooperation. These policies allow us to continue providing timely, high-quality care to all of our patients.

In-House Policies
Our goal is to provide a safe, welcoming, and professional environment for all patients and staff.
• Please arrive 10–15 minutes before your scheduled appointment if paperwork or updates are needed.
• Patients receiving aesthetic treatments should carefully follow all pre-treatment and post-treatment instructions to help ensure optimal results and reduce the risk of complications.
• Photography may occasionally be used for treatment documentation and progress tracking. Written consent will always be obtained before any photographs are used for marketing or educational purposes.
• To maintain a relaxing and professional atmosphere, we ask that cell phones be silenced during appointments whenever possible.
