Revive Dermatology Clinic & Spa Policies

We value your time and trust, and we want every experience at Revive to reflect the professionalism and care our patients expect. To help us maintain exceptional service for everyone, please review our office policies below.

Cancellation Policy

We kindly ask for at least 48 hours’ notice for all appointment cancellations or changes.
If your appointment is on a Monday, please contact us by Friday to avoid any fees.

This allows us to offer your appointment time to another patient on our waiting list.
Appointments canceled with less than 48 hours’ notice, or missed without notice, may result in a forfeited deposit or service charge.

No-Show Policy

Patients who do not show up for their scheduled appointment will forfeit their booking deposit.
After two or more no-shows, future appointments may only be scheduled at the discretion of the clinic.

We understand that emergencies happen and are happy to review situations individually — communication is key.

Refund Policy

All service sales are final once performed.
Because treatment results are based on individual response and treatment plans, we cannot offer refunds on completed services.

For safety and product integrity, we also cannot accept returns or exchanges on any skincare or retail products once purchased and removed from the clinic.

Identification & Check-In

All patients must present a valid photo ID or driver’s license at check-in for each visit.
This ensures proper identity verification and protection of your personal health information.

Medical Dermatology Appointments

Medical dermatology appointments do not require a deposit at the time of booking.
To reserve your preferred time, please call the clinic directly at 515-965-5677, as medical scheduling may differ from aesthetic appointments.

For your visit, please:
    •    Bring your insurance card to every appointment.
    •    Complete all digital intake and consent forms before arrival.
    •    Wear comfortable, loose-fitting clothing to allow your provider to easily evaluate any areas of concern.

In-Clinic Policies

To ensure a calm and professional environment for all patients:

    •    Please complete all online forms at one setting that you receive or arrive 10–15 minutes early for paperwork and                    check-in. Read any information sent about your appointment.
    •    Children under 12 must be supervised by another adult.
    •    Cell phones must remain on silent during treatments. Video recording is not permitted.

    •    Patients arriving more than 10 minutes late may need to be rescheduled at the provider’s discretion.
