Terms and Conditions
We require a 24 hour notice for cancellation with a full refund. If you cancel with less than 24 hours notice you will be charged a $75 cancellation fee. This will be refunded after you are seen if you reschedule.
When you book an appointment you will receive several forms via email that are required to be completed prior to your appointment. If these forms are not completed we will need to cancel and reschedule the appointment. Completing the forms helps ensure that you're scheduled for the appropriate service and provides us with necessary information to better deliver exceptional care.
