1. Booking Confirmation

• All bookings are subject to availability. The most updated availability should be shown through the online booking portal. 

• A booking is considered confirmed only after a confirmation email is sent.

• It is your responsibility to ensure the accuracy of the details provided during the booking process.

2. Payment

• Full or partial payment may be required at the time of booking. 

• Accepted payment methods include credit/debit cards, cash, Venmo, Affirm, Care Credit, and other secure payment gateways.

• All prices are listed in US Dollars and include applicable taxes unless otherwise stated.

• If you are NOT on a membership program, you will be charged for applicable ala cart fees based on your service type and/or products purchased.  
  • Ala cart fees may include, but are not limited to: 
    • $150 Initial Consult

    • $160 Initial Lab Fee

    • $75 Follow-up Visit

    • $95 Follow-up Lab Fee

    • $75 Lab Review

    • $50 per email or phone conversation 
    • $25 per medication refill  
    • Lab fees determined based on the number of tests being ordered. 
    • Other fees would include applicable prescriptions sent to compounding pharmacies 
• Membership billing is on a monthly, quarterly or yearly basis based on the membership purchased. Your membership starts on the first day of your membership purchase and the billing cycles run either monthly, quarterly or yearly on your purchase anniversary date. 
  • Memberships require a 30-day written notice to cancel. 
  • Membership fees are NOT refundable.
• Payment is due at the time of service and additional fees may apply for outstanding balances. 

3. Cancellation and Refund Policy

• Only cancellations made at least 24 hours prior to the scheduled booking date will be cancelled without penalty. 

• Cancellations made within 24 hours of the appointment WILL incur a cancellation fee. 

• Any visit marked as a 'No-show' WILL incur a cancellation fee. 

  • $50 for any sick visit or follow-up appointment
  • $100 for any IV infusion, Aesthetic service or new patient appointment
• Cancellation fees or 'No-show' fees will NOT be refunded. 
• There are NO refunds on services provided or received or for products purchased. There are NO refunds on ANY membership program. Membership cancellations WILL require a 30-day cancellation notice.

4. Modification of Booking

• Changes to bookings are subject to availability.

• Requests for modifications must be made at least 24 hours in advance.

• Additional charges may apply for rescheduling or upgrades done with less than 24 hours notice. 

• Please call us at 309-837-0342 to modify your appointment booking.  

5. Customer Responsibilities

• Clients must arrive on time and comply with any rules or policies associated with the booking.

• All consent forms and intake forms MUST be completed prior to the start of your visit. If necessary, please arrive 15 minutes early to ensure your forms are completed.  

• You agree not to use the service or product for any unlawful or prohibited purpose.

6. Provider Rights

•  We will do our very best to ensure timely appointments and will work with you to ensure that all of your concerns and needs are met.

• We reserve the right to cancel or modify bookings for any reason including unforeseen circumstances, without liability.

• In the event that your appointment needs to be cancelled, you will be offered an alternative date/time.
