Elevate Med Spa – Terms and Conditions & Client Agreement

By booking and receiving services at Elevate Med Spa, you acknowledge and agree to the following terms and conditions. These policies are in place to ensure a safe, respectful, and consistent experience for all clients and staff.

1. Appointments & Deposits

• A non-refundable $50 deposit is required to book all appointments. This deposit is applied toward your total service cost.

• Appointments without a deposit are considered unconfirmed and may be released.

2. Cancellation & Rescheduling

• We require at least 24 hours’ notice for any cancellations or rescheduling.

• Cancellations with less than 24 hours’ notice or no-shows will forfeit their deposit.

• Clients with repeated late cancellations or no-shows may be restricted from future bookings.

3. Late Arrival Policy

• Please arrive on time. Clients arriving 10 minutes or more late may be asked to reschedule and will lose their deposit.

• If your provider is still able to see you, your treatment time may be shortened to avoid delaying other appointments.

4. Refund Policy

• All service sales are final. No refunds will be issued for completed treatments.

• We do not guarantee specific results, as individual outcomes vary.

• Product purchases may be eligible for exchange if unopened and returned within 7 days.

5. Medical & Safety Requirements

• Clients must complete a confidential medical intake form prior to treatment.

• We reserve the right to decline or postpone services if any medical concerns or contraindications are identified.

• If under the influence of drugs or alcohol, your appointment will be canceled, and your deposit forfeited.

6. Client Conduct

• Respectful behavior is expected at all times. Abusive, inappropriate, or aggressive behavior toward staff or other clients will result in immediate dismissal and a permanent ban from our facility.

• Children under 12 must be supervised and may not accompany you into treatment rooms.

7. Privacy & Confidentiality

• All client records and personal information are kept confidential in compliance with HIPAA regulations.

• We will never share your information without your written consent.

8. Photography & Marketing

• Before-and-after photos may be taken for medical documentation. With your consent, these images may also be used for educational or marketing purposes.

9. Changes to Terms

• Elevate reserves the right to modify these terms at any time. Updates will be posted in our clinic and on our website.
