Harmony Medipa Online Booking Terms and Conditions
By booking an appointment through our online platform, you agree to the following terms and conditions. Please read them carefully before proceeding.
1. General
These terms and conditions govern your use of the online booking system for all services offered by our medical spa. The terms "we," "us," and "our" refer to the medical spa, and "you" refers to the client making the booking. We reserve the right to amend these terms at any time.
2. Eligibility
You must be at least 18 years old to book an appointment. If you are under 18, a parent or legal guardian must book the appointment on your behalf and be present during the consultation and treatment.
3. Appointment Booking
All bookings are subject to availability.
A valid credit card is required to secure your appointment. This is for our cancellation policy and will be charged at the time of booking unless otherwise stated for specific promotions or services.
By providing your contact information, you consent to receive appointment confirmations, reminders, and other communications via email or SMS.
4. Cancellation and Rescheduling Policy
We understand that unforeseen circumstances may arise. To be fair to all clients and staff, we have the following policies:
Standard Cancellation: You must notify us at least 24 hours in advance to cancel or reschedule your appointment without penalty.
Late Cancellation/No-Show: If you do not cancel or reschedule within 24 hours of your appointment or fail to show up, a cancellation fee of 50% of the service price will be charged to the card on file.
Late Arrival: Arriving more than 15 minutes late for your appointment may result in a shortened service or a need to reschedule, and the full service fee may be charged.
5. Payment
Payment for services is due in full at the time of your appointment.
We accept major credit cards, debit cards, and cash. We do not accept personal checks.
Prices for services are subject to change without notice. All prices listed on our website are for informational purposes only, and the final price will be confirmed at the time of your appointment.
6. Health and Medical Information
You must provide accurate and complete information regarding your medical history, current health status, allergies, and any medications you are taking.
Failure to disclose all relevant health information may result in an unsafe treatment, and we reserve the right to refuse service.
All information you provide will be kept confidential in accordance with our privacy policy and applicable laws, such as HIPAA.
7. Client Responsibility
You are responsible for understanding the nature of the services you are booking. If you have any questions, please contact us before your appointment.
You agree to follow all pre- and post-treatment instructions provided by our staff to ensure the best results and avoid complications.
8. Limitation of Liability
Harmony Medispa and its staff are not liable for any personal injury, loss, or damage to personal property that may occur on the premises or as a result of our services, except in cases of gross negligence.
9. Privacy Policy
Your privacy is important to us. By using our online booking system, you agree to the collection and use of your information as described in our Privacy Policy.
