Booking Terms and Conditions

By proceeding with this booking, you agree to the following terms and conditions:

1. Appointment Confirmation:
Your appointment will be confirmed via email or SMS after the booking is completed. Please ensure your contact details are accurate.
2. Cancellations & Rescheduling:
Your appointment time is reserved exclusively for you. Should you need to reschedule or cancel, we kindly request a minimum of 24 hours notice. Cancellations made within 24 hours of your scheduled visit time may incur a $50.00 late cancellation fee, at our discretion. Missed appointments without prior notice may be subject to a $100.00 no-show fee.
Applicable fees will be charged to the credit or debit card on file with RENEW Hormone Health. These fees are not billable to insurance and are the sole responsibility of the patient.

 

3. Late Arrivals:
To ensure each client receives a seamless and unrushed experience, arriving late may result in a shortened appointment time or rescheduling, depending on provider availability.  
4. Payment Terms:

Payment is due at the time of service unless otherwise agreed. We accept cash, Venmo, Zelle, & credit cards as forms of payment.
5. Medical Disclosure:
By booking, you confirm that all information provided is accurate and complete. You agree to disclose any medical conditions, medications, or allergies that may affect your care.
6. Results & Expectations:
We do not guarantee specific outcomes. Individual results may vary depending on multiple factors.
7. Eligibility:
You must be at least 18 years of age to book. Some services may be limited by medical eligibility. A consultation may be required before treatment.
8. Privacy:
All personal and medical information is kept confidential in accordance with HIPAA and applicable privacy laws.
9. Right to Refuse Service:
We reserve the right to refuse service to anyone for reasons including but not limited to health concerns, inappropriate behavior, or failure to comply with policies.
10. Changes to Terms:
We reserve the right to update these terms at any time. Continued use of our services constitutes acceptance of any changes.
