A Peaceful Environment:
Your visit to our office should be a peaceful break in your day, and we ask that you help maintain the peace by placing your cell phone on silent or airplane mode while in our office.
Children under 12:
Due to the nature of our practice and our insurance regulations, please do not bring children under 12 years of age with you to your appointment.
How we Communicate with You:
We may use your email address and your cell phone number to communicate with you. We may email and/or text reminders about upcoming appointments or notifications of events or special offers.
In return, you may call, text or email us with questions, concerns, or even send a picture of your treatment area.
In the event of an emergency, please call Kris at (850) 480-3310 or 911 if you feel that it is a life-threatening emergency.
Financial Policies:
Payment Options: We accept Cash, Care Credit, Cherry Financing and all major Credit Cards. Payment is due at the time of service. No checks please.
Missed Appointments / Late Cancellations:
We reserve a space in our schedule specifically for you. This means no one else can be seen during this time. When we have you on our schedule we will make sure to be there for you or give you as much notice as possible should we have to reschedule. We do this because we respect your time. Please respect our time as well. If you do not show up for your appointment and do not notify us at least 12 hours in advance, your card on file will be automatically charged a $100 No Show Fee. This fee is applicable to ALL appointments regardless of the length of time the appointment is scheduled for and includes follow up appointments.
Prepaid services:
Must be used within 6 months of purchase unless otherwise documented. No refunds are given for unused prepaid treatments.
Promotions:
Unless specifically stated, our promotions and/or special pricing may not be combined with other promotional offers. Please present any promotions/special pricing at the time of service; price adjustments will not be made on previous purchases or services.
Quoted Prices:
We will honor the pricing on a quote through the expiration date on that quote. Otherwise, our prices are subject to change at any time.
Follow our Pre-Treatment and Post-Treatment Instructions:
Our Pre and post treatment instructions are in place to improve the outcome and increase the safety of your treatment. You must participate in your treatment plan in order to achieve optimal results.
