By scheduling an appointment with Thryve Florida Health & Wellness, LLC (“Thryve Florida”), you agree to the following terms and conditions. Please read carefully before booking.

1. Appointment Booking

• Appointments may be booked online, by phone, or in person.
• A deposit may be required depending on the type of service booked. This will be communicated at the time of scheduling and is applied toward the total cost of your visit.
• New patient consultations must be completed prior to initiating any wellness program or therapy.

2. Payment Terms

• Thryve Florida is a cash-based practice. We do not bill insurance for services.
• All services must be paid at the time of booking or on the day of service unless otherwise arranged.
• We are happy to provide a superbill upon request, which patients may submit to their insurance provider for potential out-of-network reimbursement.

3. Cancellations & No-Show Policy

• We require at least 24 hours’ notice for all cancellations or rescheduling requests.
• Cancellations made with less than 24 hours’ notice may incur a $50 cancellation fee.
• Missed appointments without notice (“no-shows”) will be charged the full amount of the scheduled service.
• Repeated missed appointments may result in discharge from care at Thryve Florida’s discretion.

4. Arrival Time

• Please arrive at least 10 minutes prior to your scheduled appointment.
• Late arrivals may reduce the duration of your appointment or require rescheduling at your cost.

5. Packages, Memberships & Promotions

• All package and membership sales are final, non-refundable, and non-transferable.
• Cancellations of monthly subscriptions require 30 days’ notice prior to the next billing cycle.
• If you cancel mid-month, services remain active through the end of that month.
• Promotions and discounts may not be combined unless explicitly stated.

6. Concierge & Mobile Services

• In-home or mobile services are available upon request for an additional $50 concierge fee.
• Availability may vary based on provider schedule and geographic location.

7. Consent & Medical Disclosure

• By booking, you confirm that all information provided regarding your medical history and current health status is accurate to the best of your knowledge.
• Certain services may require a consultation, lab work, or provider clearance prior to being rendered.
• Thryve Florida reserves the right to decline or postpone services if it is not in the patient’s best interest.

8. Disclaimer

• Some of the therapies offered, including compounded medications and IV infusions, are not FDA-approved for all indications. These services are considered alternative wellness solutions and are elective in nature.
• By receiving care at Thryve Florida, you acknowledge that you have been informed of these distinctions and voluntarily elect to proceed.

9. Modifications

Thryve Florida Health & Wellness, LLC reserves the right to update or revise these terms at any time without prior notice. Please review this page periodically for any changes.
