Breakthrough Medical Clinic Policies

As a valued client of Breakthrough Medical, I agree to abide by the following policies:

Appointment Policies

• I acknowledge that failing to cancel or reschedule my appointment with at least 24 hours’ notice will result in a $50 no-show fee.

• Breakthrough Medical reserves the right to charge the no-show fee for appointments missed without proper notice.

• I understand that if I fail to confirm or arrive for my appointment, it may be cancelled 15 minutes after the scheduled start time.

Billing & Payments

• I agree to make regular, timely payments as scheduled for the full term of my treatment plan to keep my account current.

• If I authorize monthly recurring billing, I permit the clinic to automatically charge my credit card or bank account each month on the agreed date.

• To pause recurring billing or change the billing date, I will notify the clinic at least 72 hours in advance of the scheduled billing date.

Commitment to Care

• I will attend all scheduled appointments, whether in-person or via telehealth.

• I will comply with all medical instructions and guidelines provided by the clinic.

• I will provide accurate and complete medical information to ensure the highest quality of care.
