& Conditions
Quintessence Health & Wellness (QHW MedSpa)
These Online Booking Terms & Conditions (“Terms”)
govern all appointments scheduled through our website, booking portal, or by
phone for services provided by Quintessence Health & Wellness (“QHW
MedSpa,” “we,” “our,” or “us”). By booking an appointment with us, you (“you,”
“your,” “patient,” or “client”) agree to be bound by these Terms.
1. Appointment Booking
& Confirmation
1.1. All appointments are subject to availability and are
not confirmed until you receive a written confirmation from QHW MedSpa via
email or text.
1.2. You agree to provide accurate and up-to-date contact information at the
time of booking.
1.3. We reserve the right to reschedule or refuse appointments at our
discretion.
2. Deposits & Payments
2.1. Some services may require a non-refundable
deposit at the time of booking to secure your appointment. The deposit
amount will be applied toward your service cost.
2.2. Deposits are non-transferable unless QHW MedSpa cancels
or reschedules your appointment.
2.3. Payment for all services is due at the time of your appointment unless
otherwise arranged in writing.
2.4. Prices are subject to change without prior notice.
3. Cancellation &
No-Show Policy
3.1. We require at least 24 hours’ notice to
cancel or reschedule your appointment.
3.2. Cancellations made with less than 24 hours’ notice may result in a cancellation
fee of $50 or forfeiture of your deposit.
3.3. Failure to arrive for your scheduled appointment (“No-Show”) will result
in a full forfeiture of your deposit and may require
prepayment for future bookings.
4. Late Arrivals
4.1. If you arrive late, your appointment time may be
shortened to avoid delays for other clients.
4.2. If you are more than 15 minutes late, your appointment may be
considered a no-show and may need to be rescheduled/ and or charged a $50 late/
no-show fee.
5. Medical &
Eligibility Requirements
5.1. Some treatments require a medical
consultation, health history review, or physician approval prior to
treatment.
5.2. You must provide accurate health information. Misrepresentation or
withholding relevant information may result in denial of service without a
refund.
5.3. QHW MedSpa reserves the right to refuse treatment to anyone deemed
unsuitable for a service for safety or medical reasons.
6. Service Results &
Guarantees
6.1. QHW MedSpa does not guarantee specific results for any
treatment, as outcomes vary based on individual factors.
6.2. Any information provided by QHW MedSpa is for general educational purposes
and should not be considered medical advice unless given directly by a licensed
provider.
7. Refund Policy
7.1. All sales are final. Payments for
services, packages, and deposits are non-refundable once the
service has been rendered or the package has been purchased.
7.2. Refunds may only be issued at QHW Medspa's discretion, and/ or if in the event QHW MedSpa cancels your appointment and is unable to reschedule within a reasonable timeframe.
8. Photography &
Consent
8.1. For certain treatments, you may be asked to consent to
before-and-after photos for medical documentation purposes.
8.2. We will not use your photos for marketing without your written consent.
9. Limitation of Liability
9.1. QHW MedSpa is not liable for any adverse reactions,
injuries, or outcomes resulting from treatments unless caused by gross
negligence.
9.2. You assume full responsibility for your treatment decisions and aftercare
compliance.
10. Agreement
By booking an appointment online, over the phone, or in
person, you acknowledge that you have read, understood, and agree to
