Terms and Conditions

To ensure efficient booking, protect appointment times, and uphold fairness for both patients and staff, Humble Hydration & Wellness requires a valid credit card to be securely kept on file for all appointments and medical treatment purchases.

Policy Details

  1. Credit Card Requirement
    A valid credit or debit card is required at the time of booking to secure all appointments, including in-office treatments, mobile services, and telehealth consultations.
  2. Secure Storage
    Your card information is stored securely through our HIPAA-compliant payment processor. Staff members do not have access to your full card number once entered into the system.
  3. Charges
    • At the Time of Service: Payment is processed at checkout for all services rendered, add-ons, products, and gratuity (if applicable).
    • Please note: A 15-minute grace period is allowed for all appointments. If you arrive more than 15 minutes late or fail to show, a $50 fee will apply.
    • Late Cancellations or No-Shows: Appointments canceled or rescheduled with less than 24 hours’ notice may be charged a $50 late cancellation fee.
    • No-Show Policy: Failure to attend a scheduled appointment without notice may result in a charge equal to 100% of the scheduled service.
    • Memberships and Packages: For patients enrolled in recurring memberships or pre-paid packages, charges will process automatically per your signed agreement.
  4. Refund Policy
    All sales for medical treatments, injections, infusions, memberships, aesthetic services, and products are final.
    Due to the individualized nature of medical services and the cost of materials, no refunds will be issued for any reason once treatment has been rendered or products have been opened or administered.
    • Prepaid packages, credits, or memberships are non-refundable but may be transferred or applied toward another service at management’s discretion.
    • Refunds will not be issued for dissatisfaction with service outcomes; however, Humble Hydration & Wellness will make every reasonable effort to ensure patient satisfaction and address concerns appropriately.
  5. Authorization
    By providing a card on file, you authorize Humble Hydration & Wellness to charge the card for:
    • Services rendered
    • Late cancellation or no-show fees
    • Membership renewals or recurring payments
    • Approved product or package purchases
  6. Receipts and Communication
    Electronic receipts will be provided for all charges. Our team will make reasonable efforts to contact you before processing any charge outside of a scheduled service or signed agreement.
  7. Policy Acknowledgment
    All patients must sign this acknowledgment prior to booking or treatment. Refusal to keep a card on file may result in denial of booking privileges.
