Thank you for scheduling your appointment with Cosmetic Laser Professionals Med Spa.
We look forward to welcoming you and providing you with the highest level of care, service, and attention.
If this is your first appointment, for your convenience and to help expedite your check-in process, you will receive a Medical Intake Form. Kindly complete the form and submit it as soon as possible. Once completed, it will automatically be sent to the appropriate department. If you have already completed the form online, please inform our team member when you arrive.
If you prefer to complete the documents in our office, please plan to arrive 30–45 minutes prior to your appointment to allow sufficient time to fill out all necessary forms and releases before your scheduled visit. This helps us accommodate all of our clients and ensures appointments run on time. We value your time as much as our own.
If you have already completed the Medical Intake Form, the only additional documents you may need to sign at the office would be any required treatment consent forms.

CANCELLATION/APPOINTMENT POLICY & SPA CHECK- IN
A 24-hour notice is required for any appointment rescheduling or cancellation. Failure to provide this notice—including instances where we are unable to reach you, you do not show for your appointment, or you cancel within the 24-hour window—will result in a $25 fee added to your account or the forfeiture of one treatment from any prepaid package (including Groupon, Living Social, or any other prepaid package type).

CLP Med Spa – Spa Etiquette Policy
At CLP Med Spa, our goal is to provide a calm, professional, and relaxing environment for every client. To ensure the best possible experience for all guests and to maintain a safe clinical setting, we kindly ask that all clients observe the following etiquette guidelines.
Appointment Policy
All services are by appointment only. Please arrive 10–15 minutes before your scheduled appointment time to allow for check-in and preparation. Late arrivals may result in a shortened treatment or rescheduling to avoid disrupting other clients.
Children
For safety and liability reasons, children are not permitted in treatment rooms or to remain unattended in the lobby. If you have a scheduled appointment, please arrange childcare in advance. Our staff cannot supervise children during treatments.
Pets
While we love animals, pets are not permitted inside the spa unless they are certified service animals as defined by ADA regulations. This policy helps maintain sanitation standards and a comfortable environment for all clients.
Phone Etiquette
To maintain a relaxing environment and ensure treatments are performed safely and without interruption, we ask that all phones be silenced during your visit. Phone calls during treatments are not permitted, as they may interfere with procedures and the overall spa experience.
Guests
Unless previously arranged, guests are not permitted in treatment rooms. This allows our providers to focus fully on delivering safe and effective treatments.
Respectful Environment
CLP Med Spa is committed to providing a professional and respectful environment for both clients and staff. Disruptive behavior, including loud conversations or repeated interruptions during treatments, may result in rescheduling or refusal of service.
Cancellation & Late Policy
We kindly request 24-hour notice for cancellations or rescheduling. Late arrivals may result in a shortened service time or cancellation to accommodate the schedule of other clients.
