By booking an appointment online, you agree to these Terms and Conditions (“Terms”) and our Privacy Policy. If you do not agree, please do not schedule or use our services.

By booking an appointment online, you acknowledge that you have read, understood, and agree to these Terms and Conditions (“Terms”) and our Privacy Policy. If you do not agree, please do not schedule or use our services.
Nature of Services
Peacock Health Collective PLLC (“the Practice”) provides:
- Concierge medical and telehealth services for hormone therapy, weight management, and lifestyle optimization.
- In-person aesthetic treatments including injectables, skin rejuvenation, and regenerative procedures.
All medical and aesthetic treatments are provided by a licensed medical provider and may require consultation, review of health history, and medical clearance prior to treatment.
Medical Disclaimer
All information provided on our website, social media platforms, or booking system is for educational and informational purposes only and should not be construed as medical advice.
Booking an appointment does not establish a provider–patient relationship. A provider–patient relationship is established only after a consultation has been completed and treatment has been medically approved.
Results vary. No guarantees are made regarding outcome, longevity, or satisfaction. All procedures carry potential risks; these will be reviewed and consented to prior to treatment.
Emergency Care: Peacock Health Collective does not provide emergency services. If you experience a medical emergency, call 911 or visit your nearest emergency department.
Booking, Deposits & Cancellations
A non-refundable deposit is required to secure your appointment and will be applied to your treatment cost.
Cancellations or reschedules require at least 24 hours’ notice. Missed, late-cancel, or no-show appointments will result in forfeiture of the deposit.
Repeated no-shows or late cancellations may result in denial of future bookings at the discretion of the Practice.
Payment Terms
All payments are due at the time of service.
Accepted forms of payment include credit/debit card, HSA/FSA (where applicable), or approved financing partners (e.g., Cherry, CareCredit).
Prices are subject to change without notice.
Refunds are not provided for completed services, initiated programs or memberships, bundled services, or purchased medications.
Consent & Treatment Authorization
By booking online, you acknowledge and agree that:
- You must complete all required medical intake forms and disclose accurate and complete health history prior to treatment.
- You authorize Peacock Health Collective’s providers to perform the procedures for which you are scheduled, contingent upon medical approval.
- The provider reserves the right to modify, postpone, or decline treatment if medically contraindicated or deemed unsafe.
Telehealth Notice (for Hormone & Lifestyle Programs)
You agree to receive care through secure telehealth platforms in compliance with HIPAA regulations.
You understand that telehealth has limitations, including lack of physical examination, and that you may be referred for in-person evaluation when appropriate.
You must be physically located in Illinois or another state in which your provider is licensed at the time of the telehealth visit.
You are responsible for ensuring a private and secure environment during telehealth visits and for providing accurate location information at the time of the appointment.
Product & Prescription Policy
Compounded medications, peptides, and prescription therapies are dispensed through partner pharmacies.
All medication sales are final once the order has been processed.
Shipping fees and pharmacy timelines vary; Peacock Health Collective is not responsible for delays caused by third-party pharmacies or shipping carriers.
Liability Disclaimer
To the fullest extent permitted by law, Peacock Health Collective PLLC and its affiliates are not liable for any damages, injuries, or adverse outcomes arising from or related to your use of our website, booking system, telehealth services, or in-person treatments.
By proceeding with any treatment, you assume all known and unknown risks associated with elective medical or aesthetic care.
Privacy & Confidentiality
All personal and medical information collected through the booking platform is confidential and protected under HIPAA and applicable state privacy laws.
Photography & Marketing
Before-and-after photos may be taken for medical documentation purposes and, with your separate written consent, may be used for marketing or educational purposes.
Intellectual Property
All logos, branding, text, and images associated with Peacock Health Collective are the property of Peacock Health Collective PLLC and may not be used without written permission.
Governing Law
These Terms are governed by the laws of the State of Illinois. Any disputes will be resolved through binding arbitration in Cook County, Illinois, except where prohibited by law.
