Booking Terms

1. Credit Card Requirement
A valid credit card is required to reserve an appointment. Your card will only be charged according to the terms outlined below.

2. Fees

• Weight Loss Consultation & Lab Fee – $250
This fee will be charged when you book.  This is non-refundable and covers the cost of your initial visit and lab work. 

• Booking Fee – $50
A non-refundable booking fee is required to secure your appointment.
This amount can be applied toward products or services at your appointment.
This fee is not a separate additional charge—it is collected only if the booking is confirmed.

• Cancellation Fee – $50
If you cancel with less than 24 hours’ notice, your credit card will be charged a $50 cancellation fee.

• No-Show Fee – $50
If you do not attend your appointment without prior notice, a $50 no-show fee will be charged to your card.

3. Authorization

By booking an appointment, you authorize us to securely store your credit card information and charge your card based on the policies listed above.
