Beverly Hills Rejuvenation Center Booking & Appointment Terms
1. Appointments & Reservations
All appointments must be scheduled in advance and are subject to availability. Appointments may be booked online, by phone, or in person. We reserve the right to refuse or reschedule appointments at our discretion, including for clinical, staffing, or compliance reasons.
2. New Client Consultations
Certain treatments require a consultation prior to service. New clients may be required to complete intake forms, medical history disclosures, and consent documentation before treatment can be rendered. Failure to provide accurate or complete information may result in appointment cancellation.
3. Deposits & Prepayment
Some services may require a non-refundable deposit or full prepayment at the time of booking. Deposits are applied toward the total cost of the service and are forfeited in accordance with our cancellation policy.
4. Cancellation & Rescheduling Policy
We require at least [24] hours’ notice to cancel or reschedule an appointment.
Cancellations or reschedules made within the required notice period will not incur a fee.
Late cancellations or no-shows will result in forfeiture of the deposit.
Repeated late cancellations or no-shows may result in booking restrictions or prepayment requirements.
5. Late Arrival
Clients arriving more than [10–15] minutes late may have their appointment shortened or canceled to avoid impacting other clients. Full service fees may still apply.
6. Medical Clearance & Contraindications
Clients are responsible for disclosing all relevant medical conditions, medications, allergies, and recent procedures. Certain treatments may be contraindicated based on medical history, pregnancy, or current medications. We reserve the right to decline or modify treatment for safety or medical reasons without obligation to provide a refund.
7. Treatment Outcomes
Individual results vary and cannot be guaranteed. By booking and receiving services, clients acknowledge that aesthetic and medical outcomes differ based on individual factors and adherence to post-treatment instructions.
8. Pricing & Promotions
Prices are subject to change without notice. Promotional pricing, packages, or memberships cannot be combined unless explicitly stated. Unused services in packages or memberships are non-transferable and non-refundable unless required by law.
9. Refund Policy
All services rendered are final sale. Refunds are not provided for completed treatments. Prepaid services, packages, and deposits are non-refundable except as required by applicable law.
10. Right to Refuse Service
We reserve the right to refuse service to any client for reasons including, but not limited to, safety concerns, inappropriate behavior, non-compliance with policies, or failure to disclose medical information.
11. Consent & Acknowledgment
By booking an appointment, clients acknowledge that they have read, understood, and agree to these Booking & Appointment Terms and any additional policies, including informed consent and privacy practices.
