Booking Terms and Conditions
Effective Date: 02/15/2026
By scheduling an appointment
through the RIVIR IV Infusion Studio, LLC ("RIVIR") online booking
system, you agree to the following terms and conditions. Please read these
terms carefully before completing your booking.
1. Booking Requirements
To complete an online booking,
you must:
•
Be at least 18 years of age
•
Provide accurate and complete contact information
•
Provide a valid credit or debit card for the required
deposit
•
Agree to these Online Booking Terms and Conditions
2. Deposit Policy
A non-refundable deposit of
$50.00 is required at the time of booking to secure your appointment time slot.
This deposit will be applied toward your total service fee at the time of your
appointment.
The deposit is processed
immediately upon booking confirmation. By providing your payment information,
you authorize RIVIR to charge the deposit amount to your card on file.
3. Card on File Policy
A valid credit or debit card is
required to be kept on file for all appointments. Your card information will be
securely stored and used for:
•
Processing the required $50.00 booking deposit
•
Charging cancellation fees if applicable
•
Charging no-show fees if applicable
•
Processing payment for services rendered at the time of
your appointment
Your payment information is
processed through secure, HIPAA-compliant systems and will be handled with the
utmost confidentiality in accordance with our Privacy Policy.
4. Cancellation Policy
We understand that schedules
change. To ensure we can accommodate all clients seeking our services, we
require advance notice for cancellations.
Cancellations Made 48 Hours
or More in Advance:
Your $50.00 deposit will be credited toward a future appointment within six months of the original booking date. To reschedule, please contact us at renew@rivirspa.com or call us directly.
Cancellations Made Less Than
48 Hours in Advance:
Your $50.00 deposit will be
forfeited. The deposit is non-refundable and non-transferable when
cancellations are made with less than 48 hours notice.
How to Cancel:
To cancel your appointment, please contact us as soon as possible by email at renew@rivirspa.com or by phone. Cancellations are processed based on the date and time we receive your notice, not when you originally decided to cancel.
5. No-Show Policy
If you fail to appear for your
scheduled appointment without prior cancellation notice, you will be considered
a no-show. The following will apply:
•
Your $50.00 deposit will be forfeited and is
non-refundable
•
An additional $50.00 no-show fee will be charged to
your card on file
•
Future bookings may require full prepayment at the
discretion of RIVIR
We value your time and the
personalized care we provide. No-show appointments prevent us from serving
other clients who need our services. We appreciate your understanding and
cooperation.
6. Rescheduling Policy
You may reschedule your
appointment without penalty if you provide at least 48 hours notice. Your
$50.00 deposit will be transferred to your new appointment date.
To reschedule, please contact us at renew@rivirspa.com or call us directly. Rescheduling requests made with less than 48 hours notice will be subject to the cancellation policy stated above.
7. Late Arrival Policy
We reserve dedicated time for
each client to ensure a serene, unhurried experience. If you arrive more than
15 minutes late for your scheduled appointment, we may need to reschedule your
session to avoid disrupting subsequent appointments.
Late arrivals that result in
rescheduling will be subject to our cancellation policy based on the timing of
the rescheduled appointment. We appreciate your punctuality and understanding.
8. Provider Authorization Required
All RIVIR services require
evaluation and authorization by a licensed medical provider. Booking an
appointment does not guarantee service will be rendered. Services will only be
provided if:
•
You complete a comprehensive clinical intake and
consultation
•
A licensed medical provider determines the requested
services are clinically appropriate for you
•
There are no contraindications or safety concerns
identified during your assessment
If a provider determines that
services cannot be safely or appropriately rendered, your appointment will be
cancelled and your deposit will be refunded in full.
9. First-Time Client Requirements
If you are a first-time RIVIR
client, you will be required to complete a comprehensive wellness intake form
and consultation with a licensed medical provider prior to your first
in-residence service appointment. This consultation may occur:
•
Via telehealth prior to your scheduled appointment, or
•
In person at the time of your appointment (this may
extend your appointment time)
Please allow adequate time for
this important clinical assessment. Failure to complete the required intake and
consultation may result in rescheduling of your service appointment.
10. Service Location and Access Requirements
When booking an in-residence
service, you agree to:
•
Provide a safe, clean, and appropriate environment for
service delivery
•
Ensure our registered nurse can safely access your
location
•
Provide accurate address and access instructions (gate
codes, parking, entry details)
•
Be present at the scheduled location at the appointed
time
If our nurse is unable to
access your location or the environment is deemed unsafe or inappropriate for
service delivery, the appointment may be cancelled and will be subject to our
no-show policy.
11. Payment and Pricing
Service pricing is determined
by the provider-directed therapy plan and the specific treatments administered.
Your $50.00 deposit will be applied to your total service fee.
The remaining balance is due at
the time of service and will be charged to your card on file unless alternative
payment arrangements have been made in advance. Pricing will be confirmed at
the time of your clinical consultation and booking. All prices are subject to
change, though your rate will be honored at the price confirmed at the time of
your booking.
12. Inclement Weather and Emergency Cancellations
In the event of severe weather,
natural disasters, public health emergencies, or other circumstances beyond our
control that make it unsafe or impossible to provide services, RIVIR reserves
the right to cancel or reschedule appointments.
If RIVIR cancels your
appointment due to emergency circumstances, your deposit will be fully credited
toward a rescheduled appointment with no expiration date, or refunded at your
request.
13. Modification of Booking Terms
RIVIR reserves the right to
modify these Online Booking Terms and Conditions at any time. Any changes will
be posted with a revised effective date. Terms in effect at the time of your
booking will apply to that specific appointment.
14. Privacy and Confidentiality
All information provided during
the booking process is handled in accordance with our Privacy Policy and
applicable healthcare privacy regulations, including HIPAA. Your personal and
payment information will be kept strictly confidential.

Acknowledgment and Agreement
By completing your online
booking, you acknowledge that you have read, understood, and agree to be bound
by these Online Booking Terms and Conditions. You confirm that all information
provided is accurate and that you authorize RIVIR to charge your card on file
for the deposit, services, and any applicable fees as outlined in these terms.
