BOOKING TERMS

By booking an appointment with Rooted Functional Medicine & Longevity Lab, you acknowledge and agree to the following:

Time Zone Policy (Pacific Standard Time)

All appointments are scheduled in Pacific Standard Time (PST). Example: If you live on the East Coast and select a 3:00 PM appointment time, your session will be held at 3:00 PM PST (which is 6:00 PM EST).

Scope of Services

All services are educational and wellness-focused and are not a substitute for medical care, diagnosis, or treatment.

Cancellations & Rescheduling

Appointments must be canceled or rescheduled at least 24 hours in advance.

Late Arrivals

Late arrivals may result in a shortened session and may require rescheduling to ensure adequate time and quality of care.

No-Shows / Late Cancellations

No-shows and late cancellations may result in forfeiture of the booking fee (if there is one) and/or a rescheduling fee.

Emergency Care

This service is not for emergencies. If you are experiencing a medical emergency, call 911 immediately.

Provider Notice

Dr. Adrienne Guthrie is a Board-Certified Doctor of Natural Medicine (PhD) and Functional Medicine Practitioner. She is not an MD, DO, or ND.
