Online Booking Terms & Conditions

1. Appointment Request
Submitting an online booking request does not guarantee an appointment time. All appointments are subject to confirmation and medical eligibility.

2. Medical Assessment
All clients must complete a health questionnaire and undergo a medical screening prior to receiving the therapy of their choice. Therapy may be declined if it is determined that the therapy is not appropriate or safe.

3. Arrival Time/ Late Arrivals
Please arrive 10–15 minutes before your scheduled appointment to complete any required paperwork.
Clients arriving more than 15 minutes late may need to reschedule or have their appointment shortened, depending on provider availability. 

4. Payment
Payment is due at the time of service unless prepaid. Accepted payment methods include all major credit cards, debit cards, HSA/FSA cards (where applicable), and cash.

5. Medical Disclaimer
 All therapies at thriveMD are intended to support your wellness. It is not a substitute for emergency medical care, primary care, or treatment of serious medical conditions. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. 

6. Privacy
Your personal and medical information will be handled in accordance with applicable privacy laws and our Privacy Policy. 

7. Consent
By booking an appointment, you acknowledge that:

• The information you provide is accurate.
• You understand these booking terms.
• You agree to complete all required medical consent forms before therapy. 
• Booking an appointment does not guarantee that therapy will be administered.
