GLP-1 + B12 Program Package Terms 2-month and 6-month packages include telehealth visits and GLP-1 medication supply, offered at a discounted rate compared to month-to-month pricing. All multi-month package purchases are final and non-refundable. By purchasing a 2-month or 6-month package, you are committing to the full program term and associated pricing. Usage Window 2-Month Package: Must be used within 75 days 6-Month Package: Must be used within 222 days Any unused services or medication eligibility after this period will be forfeited without refund. Medical Eligibility Participation in the program is subject to ongoing provider evaluation and approval. Treatment, dosing, and continuation are not guaranteed. If a provider determines treatment is no longer appropriate, services may be discontinued without refund. Medication Policy Medications are prescribed at the discretion of a licensed provider. Once medication has been processed, compounded, or picked up, it cannot be returned or refunded. Appointments & Compliance Patients are responsible for completing scheduled telehealth visits within the package timeframe. Missed or delayed appointments may impact treatment and count toward included visits. We require at least 24 hours’ notice to cancel or reschedule an appointment. If you do not show up for your scheduled appointment, or if you cancel with less than 24 hours’ notice, you will be charged a $50.00 fee per occurrence. Non-Transferable Packages are non-transferable and may only be used by the original patient. Expiration All packages expire after the stated timeframe. Extensions are not guaranteed and are granted only at management discretion. Agreement By purchasing, you acknowledge and agree that this is a non-refundable, provider-guided medical program and that results and treatment continuation are not guaranteed.
