Terms and Agreements

Out of Office Aesthetic Medicine provides medical aesthetic, wellness, and related services under the supervision of licensed medical professionals. All treatments are elective and results may vary by individual. By booking services, you acknowledge that you have provided accurate medical information, understand the nature of the services, and agree to follow all pre- and post-care instructions provided by the clinic. Services rendered are personalized medical and aesthetic treatments and are not guaranteed to produce identical outcomes for every patient. Our services are not intended to replace primary medical care or emergency services.

 

Cancellation Policy

We kindly request at least 24 hours’ notice for appointment cancellations or rescheduling. Appointments canceled within 24 hours or missed appointments (no-shows) will incur a $50 cancellation fee. This fee is charged to compensate for reserved provider time and operational costs. Patients arriving more than 10–15 minutes late may need to be rescheduled and may be subject to the cancellation fee. By scheduling an appointment, you acknowledge and agree to this policy.

 

Deposit Policy

Some services require a deposit to reserve an appointment. Deposits are applied toward the total cost of the scheduled service. Deposits are non-refundable and will be forfeited if the appointment is canceled within 24 hours, rescheduled within 24 hours, or missed without notice. Deposit requirements and amounts are disclosed at the time of booking. By scheduling a deposit-based service, you acknowledge and agree to this policy.

In-House Policies

All patients must complete required medical intake forms and provide accurate health information prior to treatment. Services are elective and subject to provider assessment and medical appropriateness. Treatment results vary and are not guaranteed. For safety and privacy, additional guests or children may be restricted during medical and aesthetic procedures. The clinic reserves the right to refuse or postpone treatment if it is deemed medically inappropriate or if policies are not followed.

 

Identification Policy

For patient safety, medical compliance, and proper recordkeeping, a valid government-issued photo ID is required and must be kept on file prior to receiving medical or aesthetic services. The name on the ID must match the patient’s medical records and payment information when applicable. This policy helps ensure accurate patient identification, prevent fraud, and maintain compliance with medical documentation standards. Failure to provide a valid ID may result in rescheduling or inability to proceed with services. 

Refund Policy

All medical, aesthetic, and wellness services, packages, and memberships are final sale and non-refundable once purchased. Due to the medical nature of services and reserved provider time, refunds are not provided for services rendered, prepaid packages, or unused treatments. Exceptions may be made at the sole discretion of the clinic in accordance with applicable laws.
