Appointment Cancellation & No-Show Policy

• A minimum of 48 hours' notice is required to cancel or reschedule any appointment.
• Cancellations or rescheduling requests made with less than 48 hours' notice will incur a $50 cancellation fee.
• Patients who fail to appear for a scheduled appointment without notice may be charged a $50 no-show fee.
• Repeated late cancellations or no-shows may require a deposit to schedule future appointments.

Refund Policy

• Due to the nature of medical services and medical-grade products, all sales are final.
• No refunds will be provided for products that have been purchased, dispensed, opened, or removed from the practice.
• No refunds will be provided for services that have been performed, completed, or partially completed.
• Treatment outcomes vary among individuals, and no specific results can be guaranteed.
• Patients agree to discuss any concerns directly with HaloMedicine before seeking external remedies.

Treatment Results

• Every patient responds differently to treatment.
• No guarantees or warranties, expressed or implied, are made regarding treatment outcomes.
• Some treatments, including BHRT, GLP-1 therapy, peptides, laser treatments, skin rejuvenation procedures, and weight-loss programs, may require several weeks or months before noticeable results occur.
• Patients understand that achieving desired results may require multiple treatments, ongoing maintenance, lifestyle modifications, and adherence to medical recommendations.

Deposits & Prepaid Services

• Deposits are non-refundable but may be applied toward treatment if the appointment is kept.
• Prepaid packages and memberships are non-refundable and non-transferable unless otherwise stated in writing.
• Expiration dates on promotional offers, packages, memberships, and prepaid services will be strictly enforced.

Patient Communication Policy

• Patients are encouraged to contact the office with questions or concerns regarding treatment, billing, products, or outcomes.
• Medical concerns should be communicated directly to the practice as soon as possible to allow appropriate evaluation and management.
• Abusive, threatening, harassing, or inappropriate behavior toward staff or providers will not be tolerated and may result in dismissal from the practice.

Financial Responsibility

• Payment is due at the time services are rendered unless prior arrangements have been made.
• Patients are responsible for providing accurate billing and payment information.
• Declined credit cards, returned checks, chargebacks, and payment disputes may be subject to additional administrative fees.

Chargeback Policy

• Patients agree to contact HaloMedicine to resolve any billing concerns before initiating a credit card dispute or chargeback.
• Initiating a chargeback without first attempting to resolve the matter with the practice may result in collection efforts and recovery of administrative and legal costs where permitted by law.

Photography & Documentation

• Clinical photographs may be taken for medical documentation, treatment planning, and monitoring treatment progress.
• Photographs will remain confidential and part of the medical record unless separate written consent is provided for marketing or educational purposes.

Prescription & Medication Policy

• Prescriptions, compounded medications, peptides, BHRT products, and GLP-1 medications are prescribed solely at the provider's medical discretion.
• Medication costs are non-refundable once prescribed, ordered, dispensed, or shipped.
• Patients are responsible for following all medication instructions and reporting side effects promptly.

Arbitration & Dispute Resolution

• Any dispute arising from services rendered by HaloMedicine shall first be brought to the practice for review and an opportunity to resolve the matter.
• If a dispute cannot be resolved informally, the parties agree to submit the dispute to binding arbitration in Cook County, Illinois, rather than litigation, to the extent permitted by applicable law.
• Each party shall bear its own attorney fees and costs unless otherwise awarded by the arbitrator.

Acknowledgment

By receiving services, purchasing products, signing treatment consents, or scheduling appointments with HaloMedicine, patients acknowledge that they have read, understood, and agree to abide by these In-House Policies.
