ONLINE BOOKING TERMS & CONDITIONS

By scheduling an appointment through our online booking system, you agree to the following terms and conditions:

1. Services Offered

Revive Wellness and Aesthetics, LLC provides services including, but not limited to:

• IV Therapy
• Vitamin Injections
• Medical Weight Loss Programs
• Peptide Therapy
• Neurotoxin Treatments (e.g., Botox)
• VI Chemical Peels
• Exosome Facials
• Hair Restoration Treatments

All services are performed by qualified professionals in accordance with applicable laws and regulations.

2. Appointment Booking & Credit Card on File

• A valid credit card is required to be placed on file at the time of booking.
• By providing your card information, you authorize Revive Wellness and Aesthetics, LLC to charge applicable fees in accordance with this policy.

3. Cancellation & No-Show Policy

• We require at least 24 hours’ notice for any appointment cancellations or rescheduling.
• Failure to provide at least 24 hours’ notice will result in a $50 cancellation fee charged to the card on file.
• No-shows (missed appointments without notice) will also be charged a $50 fee.

4. Late Arrival Policy

• Clients arriving late may have their service time shortened to accommodate other scheduled appointments.
• Arrivals more than 15 minutes late may be considered a missed appointment and subject to the $50 fee.

5. Payments

• Payment in full is due at the time services are rendered.
• We accept credit card, HSA/FSA, cash/Zelle/Venmo
• No deposits are required for booking; however, a credit card on file is mandatory.

6. Refund Policy

• All services are non-refundable once performed.
• If you have concerns about your results, please notify us within 7 days so we can evaluate and address the issue.

7. Medical Disclosure & Consent

• Clients must provide complete and accurate medical history, including medications, allergies, and existing conditions.
• Certain services (including IV therapy, injectables, peptides, and medical weight loss) may require medical evaluation and approval prior to treatment.
• By booking, you acknowledge that failure to disclose relevant medical information may impact your safety and results.

8. Results Disclaimer

• Results vary from person to person and are influenced by individual health factors, adherence to treatment plans, and lifestyle.
• No guarantees are made regarding specific results.

9. Minors

• Clients under 18 years of age must have written consent from a parent or legal guardian and may require their presence during treatment.

10. Right to Refuse Service

Revive Wellness and Aesthetics, LLC reserves the right to refuse service to any individual for reasons including, but not limited to, safety concerns, medical contraindications, or inappropriate behavior.

11. Privacy

All personal and medical information collected is kept confidential and used solely for treatment, scheduling, and communication purposes in compliance with applicable privacy laws.

12. Policy Changes

Revive Wellness and Aesthetics, LLC reserves the right to modify these terms at any time. Continued use of our services constitutes acceptance of any updated terms.
