By scheduling an appointment with us, you acknowledge and agree to the following:

Appointment Requests

Online appointments are considered requests until confirmed. While our system updates in real time, we reserve the right to adjust appointments if scheduling conflicts arise.

Deposit & Card on File

A valid credit or debit card is required to reserve your appointment. Certain services may require a non-refundable deposit, which will be applied toward your treatment unless otherwise stated. No charges will be made unless our cancellation policy is violated or a deposit is required.

Cancellation Policy

We kindly request at least 48 hours' notice for cancellations or rescheduling.

Appointments canceled with less than 48 hours' notice, no-shows, or missed appointments may be subject to a cancellation fee of up to 100% of the scheduled service.

Late Arrivals

If you arrive late, we will do our best to accommodate you within your scheduled appointment time. Late arrivals may require shortening your service or rescheduling your appointment to avoid delaying other clients.

Medical Eligibility

Booking an appointment does not guarantee you are a candidate for treatment. Your provider will review your medical history and perform an assessment before treatment. Services may be modified, postponed, or declined if they are not medically appropriate or safe.

Consent Forms

Required medical history forms and consent documents must be completed prior to your appointment.

Failure to complete required paperwork may delay or require rescheduling of your appointment.

Results

Individual results vary. No specific outcome or result can be guaranteed. Recommendations provided by our providers are based on your individual goals and clinical assessment.

Minors

Clients under the age of 18 may require a parent or legal guardian to be present and provide written consent, depending on the service.

Pricing

Pricing is subject to change without notice. Quotes provided before your visit are estimates and may change if your treatment plan changes during your consultation.

Promotions

Promotions, packages, memberships, and discounts cannot be combined unless specifically stated.

Right to Refuse Service

We reserve the right to refuse or discontinue treatment if it is determined that treatment is not medically appropriate, if safety concerns exist, or if inappropriate or abusive behavior occurs.

Communication

By providing your phone number and email address, you consent to receive appointment confirmations, reminders, scheduling updates, and other service-related communications. You may opt out of marketing communications at any time.

Agreement

By booking an appointment online, you acknowledge that you have read, understood, and agree to these booking terms and conditions.
