MIRA MED SPA BOOKING TERMS

By booking your appointment, you agree to the following policies:

• A $25 deposit is required and will be applied toward your service.
• Please reply C to confirm your appointment.
• Cancellations or rescheduling requests require a minimum of 24 hours’ notice. Appointments changed or cancelled with less than 24 hours’ notice will result in forfeiture of the $25 deposit.
• No-shows will forfeit the $25 deposit. A new deposit may be required to book another appointment.
• For any appointment changes, please call MIRA MED SPA at 813-654-6472.
• Late arrivals may result in a shortened service or the need to reschedule, depending on availability.
• By booking, you acknowledge and agree to these terms.
