1. Appointment Confirmation
Submitting a booking request does not guarantee a confirmed appointment. Your appointment is confirmed only upon receipt of a confirmation email or notification from Allori Wellness. Please allow up to one business day for confirmation.
2. Consultation Types
Allori Wellness offers four consultation types: Discovery, Practitioner, Follow-Up/Check-In, and Lab Review. Each has a distinct clinical scope. By booking, you confirm you have reviewed and understand the scope of your selected consultation type. No clinical assessment, diagnosis, or prescribing occurs during a Discovery Consult.
3. Cancellation & Rescheduling Policy
Cancellations and reschedules must be made at least 24 hours prior to your scheduled appointment. Cancellations with less than 24 hours' notice will be charged the full session fee. Late arrivals may result in a shortened session. A $50 service fee applies to any returned checks.
4. Telehealth Services & Technology
Allori Wellness operates as a Florida-based telehealth practice under the Florida Telehealth Act (§ 456.47, Fla. Stat.). By booking, you consent to receiving services via telehealth and acknowledge that you may withdraw this consent at any time without affecting your right to future care. Technology interruptions may occasionally affect session quality; either party may discontinue a session if the connection is deemed insufficient.
5. Not an Emergency Service
Allori Wellness does not provide emergency medical services. In the event of a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this booking portal, email, or SMS to communicate urgent health concerns.
6. Privacy & HIPAA
All HIPAA confidentiality protections apply to your telehealth consultations. Your identifiable health information will not be disclosed to researchers or third parties without your written authorization. Standard email and SMS communications are not fully encrypted; Allori Wellness uses secure channels for clinical information whenever possible.
7. Accuracy of Information
Your provider does not have access to outside medical records or history unless you provide it directly. Withholding clinically relevant information may limit the quality of your care and could result in adverse outcomes. You agree to provide accurate and complete health information at the time of booking and during your consultation.
8. Scope of Practice
Allori Wellness specializes in peptide therapies, hormone optimization, and longevity protocols. Services are provided for wellness consulting purposes within the applicable scope of practice. Nothing in this booking or any consultation constitutes a diagnosis, cure, or treatment of any disease unless explicitly stated by your licensed provider.
9. Communication
Allori Wellness may contact you via phone, email, text message, or secure patient portal for scheduling, follow-up, and administrative matters. For time-sensitive clinical concerns between appointments, please use the patient portal. We aim to respond to non-urgent inquiries within one business day.
10. Agreement
By completing this booking, you acknowledge that you have read, understood, and agree to these Booking Terms & Disclaimer, as well as Allori Wellness's full Practice Policies & Telehealth Consent. These terms are subject to change; the most current version governs your appointment.
