ONLINE BOOKING TERMS & CONDITIONS

By scheduling an appointment online with {CompanyName}, you acknowledge and agree to the following:

  1. Appointment Requests

Submitting an online appointment request does not establish a provider-patient relationship and does not guarantee treatment, prescriptions, testing, or services.

Appointment requests are subject to provider review and approval.

  1. New Patient Consultations

All new patients are required to complete an Initial Consultation before treatment plans, prescriptions, hormone therapy, weight loss programs, peptide therapy, or other medical services can be initiated.

Consultation fees apply and are due at the time of service unless otherwise specified.

  1. Age Requirement

Patients must be at least 18 years of age to schedule appointments online unless accompanied by a parent or legal guardian where permitted by law.

Valid government-issued identification may be required.

  1. Fees and Payment

Appointment fees, consultation fees, and any required deposits are displayed during the booking process.

Payment is due at the time of service unless other arrangements have been made in advance.

  1. Cancellation and No-Show Policy

Appointments must be cancelled or rescheduled at least 24 business hours in advance.

Late cancellations and missed appointments may be subject to a cancellation or no-show fee.

  1. Medical Information

Patients are responsible for providing accurate and complete health information when requested.

Certain services may require additional medical history, laboratory testing, or provider evaluation before treatment can be provided.

  1. Provider Discretion

All treatments, medications, testing, and recommendations are provided solely at the discretion of the treating provider based on medical appropriateness and patient safety.

  1. Privacy

Information submitted through our booking system will be handled in accordance with applicable privacy laws and our Notice of Privacy Practices.
