By scheduling an appointment with iVitality Wellness MD, I acknowledge and agree to the following:

1. Appointment Scheduling: I understand that appointments are subject to availability and confirmation.

2. Cancellation & Rescheduling: I agree to notify the office at least 24 hours in advance if I need to cancel or reschedule my appointment.

3. Arrival Time: I understand that arriving late may result in a shortened appointment or the need to reschedule.

4. Medical Information: I agree to provide accurate and complete health information relevant to my treatment.

5. Treatment Eligibility: I understand that all medical and aesthetic treatments are subject to an appropriate clinical evaluation and provider approval.

6. Privacy: I acknowledge that my personal and medical information will be handled according to applicable privacy laws and the practice’s privacy policies.
