As of June 30th, 2023 a credit card on file will be required to schedule an appointment online and for new patient appointments.  A deposit may be required and will be collected at time of booking.

*No Call/ No Show
Due to an influx in no call/no show appointments a $50 fee may be charged at our discretion.  We understand there are situations that may arise, so please contact us at your earliest convenience to reschedule.

We are working hard to keep our pricing consistent. As part of that effort a 3% processing fee will apply to credit card payments beginning Jan 2nd, 2026. Debit card and cash payments will continue to be accepted with no additional charge. 

*Weight Loss Consultations
All electronic paperwork must be completed prior to appointment.  If paperwork is not completed at the time of the appointment, your consultation may be rescheduled according to the provider's schedule. Please call with any questions or concerns.
