WE ARE REQUIRED BY LAW TO PROTECT MEDICAL INFORMATION ABOUT YOU

We are required by law to protect the privacy of medical information about you and that identifies you. The medical information may be information about your past, present, or future medical condition.

We are also required by law to provide you with this Notice of Privacy Practices explaining our legal duties and privacy practices with respect to medical information. We are legally required to follow the terms of this Notice. In other words, we are only allowed to use and disclose medical information in the manner that we have described in this Notice.

We may change the terms of this Notice in the future. We reserve the right to make changes and to make the new Notice effective for all medical information that we maintain. If we make changes to the Notice, we will:

WE MAY USE AND DISCLOSE MEDICAL INFORMATION ABOUT YOU IN SEVERAL CIRCUMSTANCES

 We may use and disclose medical information about you in several circumstances, including for treatment purposes, where we share your information with healthcare providers to ensure coordinated care; for payment, which involves processing billing and payment for healthcare services; and for healthcare operations, where your information is used for quality assessment, training, and other operational needs. Additionally, we may disclose information to comply with legal requirements, report public health activities such as disease outbreaks, and share data for research purposes in a way that does not identify you. In emergency situations, we may disclose information to prevent serious harm, and we can share information with family and friends involved in your care or payment, with your consent.

Your Rights

You have the right to access and obtain a copy of your medical records, request corrections if needed, and ask for restrictions on how your information is used or disclosed. You can also request an accounting of disclosures and specify how we communicate with you. If you believe your privacy rights have been violated, you may file a complaint with our privacy officer at 305-456-0824 or the privacy officer or the U.S. Department of Health and Human Services without fear of retaliation.
