We may use and disclose medical information about you in several
circumstances, including for treatment purposes, where we share your
information with healthcare providers to ensure coordinated care; for payment,
which involves processing billing and payment for healthcare services; and for
healthcare operations, where your information is used for quality assessment,
training, and other operational needs. Additionally, we may disclose
information to comply with legal requirements, report public health activities
such as disease outbreaks, and share data for research purposes in a way that
does not identify you. In emergency situations, we may disclose information to
prevent serious harm, and we can share information with family and friends
involved in your care or payment, with your consent.
You have the right to
access and obtain a copy of your medical records, request corrections if
needed, and ask for restrictions on how your information is used or disclosed.
You can also request an accounting of disclosures and specify how we
communicate with you. If you believe your privacy rights have been violated,

privacy officer or the U.S. Department of Health and Human Services without
fear of retaliation.
