Thank you for choosing Desert Agave Wellness. All appointments are conducted personally by Jules Leverenz, RN MSN ACNP-BC — a board-certified Acute Care Nurse Practitioner with over a decade of clinical experience.

Telehealth visits are available to patients located in Arizona, Colorado, New Mexico, Nevada, and Missouri. In-person aesthetic treatments are available at our Phoenix studio by appointment only.

— For telehealth: have a quiet, private space and a working camera ready. Join your video link 5 minutes early.

— For in-person: arrive at 7325 N 16th Street, Suite 135, Phoenix, AZ 85020. Parking is available on-site.

PAYMENT

Desert Agave Wellness is a cash-pay practice. We accept all major credit cards, HSA/FSA cards, Apple Pay, Zelle, and Venmo. A 3% processing fee applies to credit card transactions. We do not bill insurance directly — we are happy to provide documentation for self-submission.
