Booking Terms & Conditions

Welcome to Peak Wellness Centers! By booking an appointment with us, you agree to the following terms and conditions. Please read them carefully.

1. Appointment Booking & Confirmation

• Appointments can be booked online via our website (www.peakwellnessillinois.com), by phone, or in person.
• A confirmation email or text will be sent upon successful booking.
• If you do not receive a confirmation, please contact us at 815-348-5868.

2. Cancellation & Rescheduling Policy

• We require at least 24 hours' notice for cancellations or rescheduling.
• Late cancellations may incur a fee of $25.
• No-shows will be charged Full Fee and may require prepayment for future bookings.

3. Late Arrivals

• If you arrive late, your session may be shortened to avoid delays for other clients.
• Full payment will still be required.

4. Payment Policy

• Payment is due at the time of service unless otherwise arranged.
• We accept Visa, Mastercard, Amex, CareCredit, Check.
• A credit card may be required to secure your booking.

5. Refund Policy

• Services are non-refundable once rendered.
• Prepaid packages or memberships may be subject to specific refund terms (contact us for details).

6. Health & Safety

• Clients must disclose any medical conditions, injuries, or allergies before treatment.
• We reserve the right to refuse service if a treatment poses a health risk.

7. Liability Waiver

• By booking a service, you acknowledge that Peak Wellness Centers and its staff are not liable for any injuries or complications resulting from treatments, except in cases of negligence.

8. Privacy Policy

• Your personal information will be kept confidential in accordance with our Privacy Policy (link to our privacy policy).

9. Minors & Consent

• Clients under 18 must have a parent or guardian present to consent to services.

10. Changes to Terms

• Peak Wellness Centers reserves the right to modify these terms at any time. Updates will be posted on our website.
